Brazil could not even have forgotten the Covid-19 for one summer ? austral. From the first week of vacation (between Christmas and New Year’s Eve), the Omicron variant appeared in the front pages of newspapers and in health services. ?Right after December 31, the influx for testing started and hasn’t stopped. Every day, at closing, we have to refuse people, ” comments the nurse who greets the public at the health clinic in the neighborhood of Santa Teresa, in the center of Rio de Janeiro.

Next to the tests, the line for vaccines is certainly less crowded, but it “also increases” according to the nurse. ?As the Omicron wave is confirmed, she adds, the booster shot is now much more followed. » In this clinic, as in the 16 other structures in the city centre, ?nearly 30% of the tests are positive, but [il n?y a] had no hospitalizations yet,? rejoices doctor Amanda Cano, who coordinates this sector for the municipality of Rio.

” You should be careful “

The Santa Teresa clinic is the only place in the neighborhood where masks are worn. Just opposite, at the Bar do Gomes, we drink beers in the sun, ostensibly turning our backs on the building. “We saw the queue, but, honestly, we don’t want to think about the Covid, so we look elsewhere, this neighborhood is so magnificent”, says Sarah (who prefers not to mention her last name) came with her family from the state of Santa Catarina, in the south of the country, to discover Rio. Santa Teresa, which we like to nickname “the Montmartre of Rio”, is experiencing an influx of tourists after almost two years. “We were told to get vaccinated, the Brazilian population did it, with awareness, differently from what I have read about Europe, so it’s still to be able to enjoy it a little now”, adds her husband, swallowing a ?bolinho de bacalhau?, the specialty of the bistro.

However, the legendary optimism of Brazilians does not reach everyone. Margareth Portela, researcher at the Oswaldo Cruz Foundation (Fiocruz) and member of the Covid-19 Observatory, has kept her eyes glued to her computer’s statistics since the curves have been going up, even though she is officially on leave. ” You should be careful, warns the researcher. It is too early to know the consequences of this recent Omicron wave and we will only really see the effects in two weeks. »

Brazil is on vacation and does not want to be told again about a devastating epidemic which has claimed more than 620,000 victims in this country of 210 million inhabitants. »

A month and a half after its detection in South Africa, the Omicron variant already represents nearly 90% of new cases of Covid-19 in France. ?Much more contagious, much less dangerous?, its impact however seems difficult to anticipate, especially as the Delta variant continues to circulate at a high level. Of the approximately 400,000 people infected every day, how many will need to be hospitalized? How many beds should be provided for in conventional hospitalization and in intensive care according to the profile of the patients? To what extent can we draw conclusions from what is happening in other countries? It is to all these questions that the modellers of the Institut Pasteur are trying to answer in an analysis posted online on Wednesday January 12, in order to specify the scenarios developed at the end of December 2021 from the first data available.

To do this, the researchers compared their curves to those traced by the virus over the past fortnight, in order to rule out all the scenarios that deviated from them and keep only the most « likely ». Under the most likely assumptions now ? a virus that is both 80% more transmissible and 80% less severe ? up to 5,200 people a day could be hospitalized at the peak of the epidemic if the current trajectory does not change. . If it appears that after the holidays the French have reduced their contacts by 10%, the peak could be reduced to 3,600 ? i.e. the level reached during the first peak of 2020 ?, or even 2,500, if this reduction is 20%.

Israel and Chile take the plunge

In reality, all Western countries hesitate. Or almost. Israel, which since the start of the pandemic has been a champion of vaccination, officially began on Monday, January 3, a campaign to administer a fourth dose to all its citizens over the age of 60. The Israeli Ministry of Health based its decision on two studies. A first, epidemiological, carried out on the 850,000 people over 60 who received the booster, showed that four months after the injection, the risk of infection was multiplied by three. Figures that the thrust of Omicron risked worsening, given the ability of the variant to escape the antibodies generated by the vaccine.

The second analysis followed 154 healthcare professionals who had received their booster before August 20, 2021 and were offered the famous fourth dose. A week later, their antibodies had increased fivefold and no serious adverse effects had been observed. The head of the Israeli vaccine committee also highlighted the context, namely “huge rate” of contamination by the new variant. On the other side of the world, Chile has also taken the plunge. On Monday January 10, it began offering a fourth dose to all immunocompromised people and, from January 1is February, will come the turn of the entire population aged over 55.

Once is not custom. The strike movement launched by the teachers’ unions for Thursday January 13 has received support beyond the sphere of education professionals. On Monday, the FCPE, the first organization of parents of students, signed the call for a strike and invited parents to mobilize on Thursday to protest against the government’s management of the health crisis at school. “To tell the Minister of National Education and the government that enough is enough”, the FCPE calls on families to “do not send the children” in class on Thursday, and to join the protests.

?Like teachers, parents can no longer endure changing protocols which are always very burdensome for students and educational teams?, writes the FCPE in its press release. The protocol relaxations announced on Monday January 10 by the Prime Minister, Jean Castex, have not changed the position « rare », according to the spokesperson for the federation of parents’ representatives. “The question is not settled” with the possibility of using three self-tests, explains Rodrigo Arenas. The difficulties in obtaining self-tests in pharmacies make parents fear persistent obstacles to complying with the protocol, and the constraint of nasopharyngeal tests, which are particularly painful for the youngest, ?continue to weigh on the children?. The solution, according to the federation, remains to ?screening children in schools with saliva tests?.

But the anger that motivates the call for mobilization goes beyond the health protocol alone. Parents join the Minister of National Education on the need to keep schools open, in the interest of students. ?But opening the school doors is not enough,? says Rodrigo Arenas, denouncing the current situation in which classes are half empty and children, regularly absent, suffer in their learning.

A strike difficult to discredit

The challenge for the FCPE, as for the teachers, is that of the means given to the school to welcome the students and ensure its teaching mission in good conditions despite the health crisis. A “whatever the cost” declined to the school world to get out of the implicit alternative of the health protocol: test or close. For Rodrigo Arenas, this implies better security for classes to avoid contamination, means to ensure the replacement of all absent teachers and “not just for daycare”, or even solutions for distance education of students forced to stay at home while their teacher is in class.
